1. Is the manufacturer registered with NHTSA as a vehicle manufacturer under 49 CFR Part 566, with current registration on file?
2. Is the self-certification process documented, confirming that each applicable FMVSS standard has been reviewed, a compliance basis established, and evidence retained?
3. Are certification labels produced to the format required by 49 CFR Part 567 — including GVWR, GAWR, tire and rim size, and certification statement — affixed in the required location?
4. Is the VIN assigned using a World Manufacturer Identifier (WMI) registered with NHTSA or the designated ISO organisation, with VIN structure compliant with 49 CFR Part 565?
5. Is a batch of certification labels and VIN assignments traceable back to the production record for each vehicle, confirming the label reflects the vehicle's actual specifications?
6. Is the Registered Importer process followed for vehicles not originally certified for the US market, with proper HS-7 declaration and bond posted before import?
7. Are vehicle alterations performed after initial certification (upfitters, converters) managed through the Alteration of Certified Vehicles process under 49 CFR Part 567, with alteration labels affixed?
8. Are records of production data, certification basis, and test results maintained for a minimum of 5 years or as otherwise required by NHTSA regulations?
9. Is a quality assurance process in place to confirm that production vehicles conform to the certified design — catching manufacturing variation that could create an FMVSS noncompliance?
10. Is a defect investigation process in place, covering how potential defects are identified, assessed for safety risk, and whether the NHTSA reporting threshold has been met?
